I concur this.
I have experienced cases, when I was thrown out of a portal to different systems and planets, while dialing the same address.
This is especially important to note, when portal is saying “atlas transport error” - result could vary and player may go to the specific system and planet each time or may end up in random places.

Well I had a visitor last night which was awesome and a communication station and monument was left but unfortunately my game crashes every time I try to go near my base 
PS4 error code CE-34878-0
If I remember correctly, @Mad-Hatter had his base glitch after making a monument with @toddumptious. In that case, the entire base disappeared for visitors.
Unfortunately, a fix was never found and the base was moved.

Yes thats true.
Mr Todd (on survival) met me (on normal) and we made a monument nearby to a comm.
My game continually crashed until I eventually loaded offline and moved away.
I know it had something to so with proximity between the monument and comm, because the monument would deliver the comm’s message.
Only solution is to move and never return.

No worries, just keep spaceshipping. I finally got my Theta warp so I’m going to cruise the blue systems now.

If anyone gets a chance could they please check this portal works? The planet is Ryloth in the Even Further Outer Rim system (PS4 normal mode).

Sorry to be the bearer of bad news…
That address came up as corrupted but we tried it anyway.
Ended up in the correct system but on the frozen planet of ‘Amheuengarna’.
I’d suggest you find a single planet system to inhabit: Even corrupted portal addresses still bring you out to the only planet in that system.
I know how you feel, I copped several dud portals in a row once. Very annoying.
Edit: I was in PS4 normal but could not see a beacon or any comms.

Have you tried turning on frame rate lock to avoid crashing at bases?

Damn it, that was a lovely planet too. I might just go and live on the ice planet for now.

PC Creative Mode Euclid. I am afraid the base is a long way from the portal. This is my first attempt at a creative base.

I noticed earlier you didn’t have a mic. But if you’ve a cell phone headset it will work with your PS4.
