Apparently, the main decision to fly crewed with the heatshield as is is because they tested behaviour with only a partial shield, and it held. In other words, even with a heatshield damaged like the last one was, the astronauts would have survived, and would still if the shield was more damaged. I guess they could test that hypothesis relatively reliably on the ground.
