I think you will be able to select multiplayer or single when you load your game, I also believe there will be an invite feature so you can roll co-op without actually going full mp. Again pure speculation from me

Best case scenario they completely separate PvP and PvE entirely…keep PvP to an isolated game mode.

No, it says you need an Xbox Live Account…that does not necessarily mean you need gold…big difference…an XBL account is free.
